The most recent studies and literature continue to point towards intervention in care transition as a largely contributing factor in reduction of readmission rates.

In our proactive and outcomes-driven approach to post-acute patient care, we aim to involve the right individuals at the right time and close the communication loop. We will notify primary care physicians following patient discharge, and coordinate a follow-up visit primary care office visit within three days of discharge.

Platinum Healthcare’s EMR software, Kinnser, will grant primary care physicians the ability to securely access patient electronic charts at their convenience to better manage their patient’s care.
